Doesn't look like the pass was long enough to have fallen asleep. Probably distracted operating. Doesn't even look like there was an attempt to steer away from the water. Hope the operator survived.

Not sure origin of those pics.
But neighbour had exact thing happen to his young son. He almost drowned. Couldn't open door. Modified tank made for very little room between tank and back of cab. Had trouble finding something to smash window with in neck deep in water. Can't swing your arm under water. Case has factory glass breakers hanging in cab and now I know why.
Local case was not phone distraction.
New field. Lot of sloughs. Water level with field surface. Simple mistake. Operator was 18 I think. On the phone with Dad while chin deep trying to get out, panicking. Dad didn't know what he was gonna find when he got out there. Cab fully submerged. Kid standing on tank.
Looks funny I know, but nobody here laughed.
This spring an elderly man died when he put his track hoe in a dug out on new land.
